If you know someone who’s been an inspiration to others after suffering a stroke, you can nominate them, whether they're a stroke survivor, carer, volunteer, stroke club or group, fundraiser or a professional.
Entries must be submitted by Friday, May 24, at stroke.org.uk/life-after-stroke-awards.
Judges at this year’s event include actor Geoffrey Palmer, and former Who Wants to be a Millionaire host, Chris Tarrant OBE.
